Description of requirement and submittal requirements are below Description: The Contractor shall provide for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) Emergency Housing program provides care, treatment and rehabilitative services to homeless Veterans in Stanislaus County, CA. The services provided to eligible homeless veterans include outreach, case management and housing services in accordance with the PWS. Reference the draft PWS for additional details. Background
The Department of Veterans Affairs is seeking to provide Homeless Veterans with Contracted Emergency Residential Services (CERS) in Stanislaus County, CA. The goal of this contract is to deliver care, treatment, and rehabilitative services to homeless veterans, ensuring they receive the necessary support for transitioning from homelessness to stable housing.

This Sources Sought announcement is intended for market research purposes only and does not constitute a solicitation or request for proposals.

Work Details
The Contractor shall provide emergency housing and supportive services for homeless Veterans according to the HCHV CERS model requirements. Services include: outreach, case management, housing services, secure housing accommodations for males and females, 24-hour service availability, provision of three daily nutritious meals, laundry facilities, secure storage for personal belongings and medications, quality case management using a Recovery Model approach, and maintaining a minimum occupancy level of 80%.

The contractor is also responsible for care planning within 14 days of admission and ensuring exits to independent housing are monitored as indicators of program quality.
