Background
The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is seeking to provide Health Care for Homeless Veterans (HCHV) emergency residential services at a licensed Residential Care Facility for the Elderly (RCFE).
The goal of this contract is to ensure that homeless veterans receive secure housing and supportive services as part of their transition to permanent housing.

Work Details
The contractor shall provide emergency housing and supportive services for homeless Veterans in accordance with the HCHV CERS model requirements. Services include:
- Safe, secure housing with shared laundry and restroom facilities.
- Three daily nutritious meals, including accommodations for special dietary needs.
- Case management and care coordination services.
- Transportation services.
- Enhancement of independent living skills.
- Permanent/transitional housing search support.
- On-site care for Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
The contractor must maintain a minimum occupancy level of 80% and engage veterans in case management meetings at least three times per month.

Period of Performance
The contract will have multiple ordering periods starting from September 30, 2026, through September 29, 2031.

Place of Performance
Services will be performed in Contra Costa Counties, Solano, Yolo, Sacramento, and West Placer Counties.

Bidder Requirements
Contractors must have a current California state RCFE license for Adult and Senior Care Facility. Key personnel requirements include:
- One administrative staff member on duty 24/7;
- Sufficient case management personnel with experience working with homeless individuals;
- At least one staff member CPR certified available during each shift;
- The ability to provide resumes for key personnel upon request.
