The Department of Veterans Affairs (VA) Network Contracting Office (NCO 21) is soliciting for vendors, regardless of business size, that can provide Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) for the San Francisco VA Health Care System (SFVAHCS) in San Francisco, as described in the attached solicitation and services in the Performance Work Statement. Background
The Department of Veterans Affairs (VA) Network Contracting Office (NCO 21) is seeking vendors to provide Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) for the San Francisco VA Health Care System.

The HCHV Program aims to reduce homelessness among Veterans by providing access to supportive services and housing. The goal of this contract is to facilitate the transition of homeless Veterans into stable housing environments through immediate residential care and treatment.

Work Details
The contractor will provide short-term residential care and treatment for eligible Veterans requiring immediate housing placement. Services include:
1. Providing 10 beds per day on a per diem basis, accommodating up to 3,650 days annually.
2. Supporting Veterans in recovery through individual and group sessions addressing mental health and substance use disorders.
3. Offering case management services, including assistance with permanent housing searches, income support, and linkage to healthcare.
4. Ensuring a drug-free environment while incorporating harm reduction strategies.
5. Conducting outreach to homeless Veterans and facilitating their entry into the program without barriers related to substance use or legal history.
6. Developing individualized service plans within 14 days of admission, with regular updates every 60 days.
7. Providing secure medication storage and personal belongings storage for participants.

Period of Performance
The contract period spans from September 1, 2025, to August 31, 2030, with five ordering periods of one year each.

Place of Performance
Services will be performed in counties within the San Francisco VA Health Care System catchment area: Lake County, Marin County, Mendocino County, San Francisco County, or Sonoma County.
