Background
The Department of Veterans Affairs Pacific Islands Health Care System (VAPIHCS) is seeking to provide Health Care for Homeless Veterans (HCHV) through contracted emergency residential services.

The goal of this contract is to offer care, treatment, and rehabilitative services to homeless veterans in the Pacific Region, ensuring they have access to safe residential settings while addressing their immediate needs and facilitating their transition to permanent housing.

Work Details
The contractor will provide emergency residential services for homeless veterans, including:
1. Providing 6 beds per day on a per diem basis for eligible homeless veterans, totaling approximately 2,190 days of service per ordering period.
2. Delivering outreach, case management, and housing services aimed at stabilizing veterans' medical, mental health, and psychosocial issues.
3. Ensuring that the environment is safe and supportive for veterans with co-morbid conditions such as mental illnesses and substance abuse disorders.
4. Conducting initial assessments to verify homelessness or imminent risk of homelessness by a VA Liaison.
5. Maintaining an average occupancy rate of at least 80% for the beds provided.
6. Offering structured group activities at least twice a month to promote engagement and support among residents.

Period of Performance
The contract has an Indefinite Delivery Indefinite Quantity (IDIQ) period from September 30, 2025, to September 29, 2030, with five ordering periods spanning one year each.
