This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: 36C257-23-Q-0136 The solicitation type: Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01 Effective Date 12/30/2022 and VAAR Update 2008-34 Effective Date 12/21/2022. This combined synopsis/solicitation is issued as Unrestricted, insufficient Small Business. The associated NAICS code is 624221 Temporary Shelters, and the small business size standard is $12 million. A list of line item number(s) and items, quantities, and units of measure (including option(s)) is included under section Price Schedule. Description of services to be acquired for this contract action is provided in the Performance Work Statement (PWS). The provision at 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition. Addenda is included with the provision. The provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, specific evaluation criteria to be included in paragraph (a) of that provision. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with their quote. The clause at 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition, as well as addenda to the clause. Background
The Central Texas Veterans Health Care System (CTVHCS), Healthcare for Homeless Veterans (HCHV) program, 1901 Veterans Memorial Drive, Temple, Texas 76504, has a requirement to provide community-based Low Demand Safe Haven (LDSH) model supportive transitional housing for up to fifteen (15) homeless Veterans (both male and female) within the city limits of Austin, Texas. The period of performance of the resulting Contract will be for a base year and four (4) one-year option periods. This contract represents a Firm, Fixed-Priced, Indefinite Delivery-Indefinite Quantity contract.

Work Details
The Contractor shall provide Low Demand Safe Haven (LDSH) model supportive residential treatment services for up to fifteen (15) homeless Veterans within Austin, TX. The services include providing transitional housing for hard-to-reach homeless persons suffering from severe mental illness, substance abuse disorders, and inability to participate in more traditional therapeutic homeless housing arrangements. Other services include instruction in and assistance with health and personal hygiene, scheduled visits with each Veteran resident, discharge planning, supportive services such as assistance in obtaining a driver’s license and completing applications for public assistance/services, referral process, emergency housing facilities and management ensuring safety and sober environment requirement.

Period of Performance
The period of performance will be for a base year and four one-year option periods. The contract type is Firm, Fixed-Priced, Indefinite Delivery-Indefinite Quantity.

Place of Performance
Work will be performed at the Contractor’s facility within the city limits of Austin, TX.
