Emergency Housing COED Veterans (HCHV) San Mateo County   3

Background
The Department of Veterans Affairs Palo Alto Health Care System (VAPAHCS) located at 795 Willow Rd. Menlo Park, CA 94025 requires a contractor to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. This contract represents a Firm Fixed-Priced, Indefinite Delivery-Indefinite Quantity contract.
Costs not incorporated into the Contractor’s price will NOT be reimbursed by the Government.

Work Details
The Contractor shall provide all labor, supervision, housing, material and supplies necessary to provide emergency residential treatment, rehabilitative and supportive services. Services will be provided on-site at the Contractor’s facility, all provisions and requirements listed herein.
The prices provided in the Price Schedule shall be inclusive of all “basic services” as may be necessary in the treatment of the Veteran.

Period of Performance
IDIQ Period of Performance: 4/01/2024 - 3/31/2029; Ordering Period One: 4/01/2024- 3/31/2025; Ordering Period Two: 4/01/2025 - 3/31/2026; Ordering Period Three: 4/01/2026 - 3/31/2027; Ordering Period Four: 4/01/2027 - 3/31/2028; Ordering Period Five: 4/01/2028 - 3/31/2029
