Solicitation Emergency Housing Geriatric Veterans (HCHV)   3

Background
The Department of Veterans Affairs Palo Alto Health Care System (VAPAHCS) requires a contractor to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. This contract represents a Firm Fixed-Priced, Indefinite Delivery-Indefinite Quantity contract. Costs not incorporated into the Contractor’s price will NOT be reimbursed by the Government. This contract is specifically for the geriatric population which has additional requirements.

Work Details
The Contractor shall provide all labor, supervision, housing, material and supplies necessary to provide emergency residential treatment, rehabilitative and supportive services. Services will be provided on-site at the Contractor’s facility, all provisions and requirements listed herein.

The prices provided in the Price Schedule shall be inclusive of all “basic services” as may be necessary in the treatment of the Veteran. The Contractor will be required to provide a low-barrier therapeutic and rehabilitative milieu and attendant services targeting the underlying factors contributing to homelessness within the geriatric population.

IDIQ Period of Performance: 5/01/2024 - 4/30/2029; Ordering Period One: 5/01/2024- 4/30/2025; Ordering Period Two: 5/01/2025 - 4/30/2026; Ordering Period Three: 5/01/2026 - 4/30/2027; Ordering Period Four: 5/01/2027 - 4/30/2028; Ordering Period Five: 5/01/2028 - 4/30/2029
