The Department of Veterans Affairs, Network Contracting Office (NCO) 21, has a requirement for Health Care for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) for Homeless Geriatric Veterans in San Mateo, Santa Clara, or Southern Alameda County for VA Palo Alto Health Care System (VAPAHCS) in accordance with the performance work statement contained herein. The solicitation does not have a set aside. The North American Industry Classification Systems (NAICS) is 624221, size standard $13.5M. Contractors must be registered in the System for Award Management (SAM) at www.sam.gov to be eligible for award. Carefully read FAR 52.212-1 Instructions to Offerors and VAAR 852.273-73 Evaluation - Health-Care Resources in order to respond to this RFQ. Failure to do so may eliminate your response from being considered for award. IAW FAR 16.504, the Government intends to award a Single Award Indefinite Delivery Indefinite Quantity (IDIQ) Contract as a result of this solicitation. Background
The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is seeking to provide Health Care for Homeless Veterans (HCHV) through contracted Emergency Residential Services (CERS) specifically for homeless geriatric veterans in San Mateo, Santa Clara, or Southern Alameda County.

The goal of this contract is to remove homeless veterans from unsuitable living conditions and place them in community-based residential environments that offer supportive services to improve their housing situation. This initiative aligns with the VA's mission to support veterans' health and well-being.

Work Details
The contractor will provide all necessary labor, supervision, housing, materials, and supplies to deliver emergency residential treatment and supportive services. Key tasks include:
1. **Residential Room and Board**: Accessible 24/7 with single beds, personal linens, furnishings like dressers and storage lockers. At least 50% of beds must be wheelchair accessible.
2. **Dietetic Services**: Provision of three nutritious meals daily with dietary accommodations as needed.
3. **Laundry Facilities**: Free access to laundry facilities at least once a week.
4. **Case Management Services**: A minimum staffing ratio of one full-time medical case manager per 12 veterans is required. Case managers must provide intake packets within 72 hours of admission.
5. **Therapeutic Environment**: The facility must maintain a low-barrier therapeutic milieu that addresses the underlying factors contributing to homelessness among geriatric veterans without requiring detoxification or hospital-level care.
