This contract requires the Contractor(s) to provide a 24-hours a day / 7 days a week (24/7) community-based early recovery model through a Safe Haven model of residential care at a facility accessible by public transportation that serves homeless Veterans, including those with serious mental illness, physical disabilities, and substance use disorders. The VAMC requires two separate programs that serve two distinct levels of Healthcare for Homeless Veterans (HCHV) Contracted Residential Services (CRS), as outlined below in accordance with VHA Handbook 1162.09. The two levels are designated for the Low Demand Safe Haven (LDSH or Safe Haven) program and the Contracted Emergency Residential Services (CERS) program with details are described in the Performance Work Statement. The Contractor must have an acceptable facility within the City of Philadelphia for the Philadelphia VAMC and an acceptable facility within Delaware, Chester, or Montgomery Counties for the Coatesville VAMC. The contract facility and associated onsite services are expected to pass inspection and become fully operational within 60 days after the date of contract award.
Background
The Department of Veterans Affairs is seeking to award a five-year firm fixed price Indefinite Delivery/Indefinite Quantity (IDIQ) contract for the Healthcare for Homeless Veterans (HCHV) Program services. This contract will support both the Philadelphia VAMC and the Coatesville VAMC, aiming to provide essential services to homeless Veterans, including those with serious mental illness, physical disabilities, and substance use disorders.

The goal is to create a community-based early recovery model through a Safe Haven model of residential care that is accessible by public transportation.

Work Details
The Contractor will provide a 24/7 community-based early recovery model through a Safe Haven model of residential care. This includes:
1. Emergency residential placement and treatment services for a total of 90 homeless Veterans, with 45 veterans served by each VAMC.
2. Establishment of two separate programs: Low Demand Safe Haven (LDSH) and Contracted Emergency Residential Services (CERS).
3. Provision of onsite treatment case management services to address complex needs of Veterans, including stabilization of medical, mental health, substance abuse, and psychosocial problems.
4. Facilities must meet criteria established by HCHV clinical staff and VA fire and safety officials.
5. The Contractor must ensure that facilities in Philadelphia and surrounding counties are operational within 60 days post-award.

Period of Performance
The contract will be effective from approximately October 1, 2025, through September 30, 2030.

Place of Performance
The contract facilities must be located within the City of Philadelphia for the Philadelphia VAMC and within Delaware, Chester, or Montgomery Counties for the Coatesville VAMC.
