Healthcare for Homeless Veterans (HCHV) Recuperative Care Transitional Housing Services for VA San Diego Healthcare System (VASDHS)   3

Background
This new requirement is for HCHV Recuperative Care Transitional Housing Services to Veterans in community-based facilities for the VA San Diego Healthcare System (VASDHS). The HCHV Program is an essential and critical part of the Veterans Health Administration (VHA), providing a gateway to VA services and community resources for homeless Veterans. The aim is to engage homeless Veterans in treatment and aid them with placement in a safe, secure environment that promotes their rehabilitation goals.

Work Details
The Contractor shall provide sufficient facilities to accommodate 12 Veteran residents, including same-day residential placement, a registered nurse for no less than 35 hours per week, case management services for 40 hours per week, medication management staff 24 hours a day for seven days of the week. The services shall be available to both male and female Veterans who require recuperative medical care and/or stabilization due to sickness, injury or recovery after inpatient care provided by the VASDHS or other local community hospital. The Contractor's personnel shall provide therapeutic, rehabilitative, and recovery services determined according to the needs of the Veteran resident through an intake assessment.

Period of Performance
The contract period of performance for this IDIQ contract shall be for a base year with the option to exercise four one-year option periods. The base year period of performance is from 09/26/2023 to 09/25/2024.

Place of Performance
The priority service location shall be performed at the contractor’s facility within 40 miles from the VASDHS located at 3350 La Jolla Village Drive San Diego, CA 92161.
