The following verbiage is hereby added to update PWS section 8.5.1 REFERRAL/ADMISSION per recent VHA policy change dated October, 23, 2023: Contractors must ensure timely, same-day whenever possible, admissions to their program in accordance with VA policy. Program admission is defined as the time from when the referred Veteran is ready to enter transitional housing to the actual placement in an available bed with the goal of same-day admission or no more than 72 hours. If a Veteran is directly admitted to the Contractor s facility prior to engagement by VA homeless program staff, the Homeless Operations Management and Evaluation System (HOMES) assessment Interview must be completed within 14 days of admission, consistent with existing CRS and HOMES policies (HOMES Reporting Policy). VA staff may complete parts of the admission process to expedite admissions. Per VHA Directive 1162.04 CRS, HCHV liaisons must verify Veteran s eligibility for program participation within one business day from admission. In the event, that a Veteran is admitted to an HCHV CRS program but subsequently found to be ineligible, the HCHV CRS liaison must immediately work with the Veteran to secure other placement within available VA or community programs. VA will pay for a maximum of four days from the day of admission to allow the provider and HCHV CRS liaison time to locate and arrange an alternate placement.

Background
The Department of Veterans Affairs (VA) requires contractor(s) to provide emergency residential housing and case management services, in currently active residential treatment facilities, to homeless Veterans as part of the Community Based Health Care for Homeless Veterans (HCHV) program. The program/services are for Veterans who are eligible for VA health care and referred by one of the following VA Medical Centers. The VA has set a zero-tolerance policy for homelessness within the Veteran population and has been increasing both capacity of existing programs and services offered to program participants. The plan calls for utilizing new models of care not previously offered by VA.

Work Details
The contractor shall provide emergency residential housing and case management services for eligible Veterans referred by authorized officials at the VA Medical Center. The contractor is responsible for providing a safe and secure environment to support Veterans’ rehabilitation and recovery goals, collaborating with a variety of VA programs and services, ensuring same-day admissions or no more than 72 hours, completing HOMES assessment interviews within 14 days of admission, verifying Veteran's eligibility within one business day from admission, securing alternate placement if a Veteran is found to be ineligible after admission, and providing discharge planning and referrals for each Veteran. The contractor must also employ sufficient personnel to carry out the policies, responsibilities, and programs, including key personnel such as administrative staff, clinical supervisor, case management personnel, and staff members with CPR certification.

Period of Performance
The contract period includes a base year from January 1, 2024 through December 31, 2024, with four option years extending through January 1, 2028. Services will be provided on-site at the contractor’s facility throughout this period.

Place of Performance
Services shall be provided at the contractor’s facility located in Albany Stratton VA Medical Center, Syracuse VA Medical Center, VA Finger Lakes Healthcare System (Bath VAMC), VA Finger Lakes Healthcare System (Canandaigua VAMC), and VA Western New York Healthcare System.
