Background
The Department of Veterans Affairs (VA) is issuing this solicitation to provide Health Care for Homeless Veterans (HCHV) Services, specifically through a Contracted Emergency Residential Services (CERS) model. The goal is to support homeless veterans by providing them with immediate placement in safe environments with onsite supportive services.

This initiative aligns with the VA's mission to end homelessness among veterans, as outlined in their Plan to End Homelessness among Veterans.

Work Details
The contractor will provide emergency residential services for up to 25 homeless veterans at a facility located within 25 miles of the St. Louis VA Medical Center John Cochran Division. Key tasks include:
1. Providing time-limited services such as mental health stabilization, substance use disorder treatment, and vocational training.
2. Facilitating referrals and placements into permanent housing or longer-term residential programs.
3. Ensuring compliance with safety codes and ADA guidelines for accessibility.
4. Conducting regular inspections and maintaining a safe environment for veterans.
5. Offering therapeutic and rehabilitative services, including daily structured activities and professional counseling.
6. Coordinating with VA staff for case management and treatment plans.
7. Reporting any critical incidents involving veterans promptly to the VA Liaison.

Period of Performance
The period of performance for this contract is one base year from November 26, 2025, through November 25, 2026, with four one-year option periods extending through November 25, 2030.

Place of Performance
Services will be performed at the contractor's facility located within 25 miles of the St. Louis VA Medical Center John Cochran Division.
