HEALTHCARE FOR HOMELESS VETERANS WITH MEDICALLY ENHANCED SERVICES

Background
The Central Texas VA Healthcare System (CTXVAHCS) has a requirement for Contracted Emergency Residential Services (CERS) with medically enhanced services for up to 10 veterans (male and female) in Austin, Texas. The Health Care for Homeless Veterans (HCHV) program is an essential and critical part of VHA, providing a gateway to VA and community-based supportive services for eligible Veterans who are homeless.

Work Details
The HCHV Contracted Emergency Residential Services (CERS) Program exists to provide a means of removing homeless Veterans from the street or habitation unfit for humans and placing them in community-based, residential environments with sufficient supportive services to meet their basic needs and ultimately, facilitate the improvement of their overall health and housing situation. The program targets and prioritizes homeless Veterans who are eligible for VA health care, transitioning from literal street homelessness, being discharged from institutions, or recently becoming homeless. It provides safe, secure housing as well as supportive services including three daily nutritious meals, case management and care coordination services, vocational services, benefits services, enhancement of independent living skills, permanent/transitional housing search support, medical services and oversight services, substance overdose education and prevention protocols, internet access and computer access. The contract requires the Contractor to provide emergency housing and supportive services in Austin, Texas for up to 10 homeless Veterans who are also recovering from a medical illness or injury for the Central Texas VA Healthcare System.
