G-004 Homeless Care for Homeless Veterans (HCHV) for Saginaw, MI

Background
The Aleda E Lutz VA Medical Center in Saginaw, Michigan, is seeking to establish a community-based residential program for homeless veterans through the Healthcare for Homeless Veterans (HCHV) program. The goal of this contract is to provide rehabilitative services and shelter for homeless veterans who qualify for VA healthcare, ensuring they have access to a safe and supportive environment that aids in their rehabilitation.

Work Details
The contractor will provide HCHV rehabilitative services for no more than ten (10) homeless male veterans in the Saginaw County area. Services include:
- Therapeutic case management services, excluding detoxification or hospital-level treatment.
- Provision of three nutritionally adequate meals per day and snacks, with accommodations for special dietary needs.
- Maintenance of a clean and safe facility that meets local health and safety codes, including fire safety requirements.
- Assistance with local transportation to appointments and meetings.
- Maintenance of individual clinical records for each veteran, ensuring confidentiality and compliance with relevant laws.
- Engagement in ongoing discharge planning to facilitate successful transitions to independent living.

Period of Performance
The contract will be performed over a base year starting October 1, 2025, with four optional twelve-month extensions at the discretion of the Federal Government.

Place of Performance
The services will be performed within the city limits of Saginaw, Michigan.
