The purpose of Amendment 0004 is as follows:

1. To incorporate the changes made to FAR 52.212-2 EVALUATION - COMMERCIAL ITEMS (OCT 2014).

2. To incorporate the changes made to ADDENDUM TO FAR 52.212-1.

The purpose of amendment 0003 is to incorporate the provisions at DFARS 252.204-7016, Covered Defense Telecommunications Equipment or Services - Representations and DFARS 252.204-7017, Prohibition on the Acquisition of Covered Defense Telecommunications Equipment or Services - Representation, and the clause at DFARS 252.204-7018, Prohibition on the Acquisition of Covered Defense Telecommunications Equipment or Services.

5. incorporate the following changes made to ADDENDUM TO FAR 52.212-1 paragraph 4. PROPOSAL

SUBMITTAL REQUIREMENTS.

SOLICITATION

The contractor shall furnish all labor, equipment, materials, transportation, supervision, and any other

necessary work to accomplish the requirements for Facilities Services at the U.S. Naval Air Facility, Atsugi and

Combined Arms Training Center, Fuji, Japan in accordance with the terms and conditions.

The contractor shall provide the following services:

Grounds Maintenance

Custodial Service

Pool Maintenance

Building System Maintenance and Cleaning

Pest Control
