The purpose of this amendment is to provide answers to the questions made by vendors in response to solicitation no. 36C24823Q0241. The questions made in response to the aforementioned solicitation, as well as the answers are as follows: 1- Question: What is the pick-up frequency of the Regulated Medical Waste (RMW) for the VA Outpatient and Rural clinics? Answer: RMW pick-ups at the VA Outpatient and Rural clinics Weekly. 2- Question: Of the RMW tonnage discarded on an annual basis, what is the estimate of pathological waste? Answer: Pathological waste generated at the VACHS main facility in San Juan, Puerto Rico, is approximately 200 lbs/month (1.2 ton/year). There are no further changes to this solicitation.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. The solicitation number 36C24823Q0241 is issued as a Request for Quotation (RFQ). The incorporated provisions and clauses in this document are those in effect through Federal Acquisition Circular 2023-01, effective on 12/30/2022. This solicitation is being procured as unrestricted. Therefore, it shall be competed among all businesses (Small and Large). The associated North American Industrial Classification System (NAICS) code for this procurement is 562112, with a small business size standard of $41.5 Million. The PSC is F108. The Department of Veterans Affairs, Network Contracting Office - 8 located at VA Caribbean Health System (VACHS), 10 Casia Street, Admin Building Suite 4M440, San Juan, Puerto Rico 00921 is seeking for a Contractor to provide collection, transporting, and disposition services for Regulated Medical Waste (RMW) at the VA Caribbean Health System (VACHS). These services will be provided at the VACHS main facility in San Juan, the Ponce Out-patient Clinic (POPC), the Mayaguez Out-patient Clinic (MOPC), the Guayama Community-Based Out-patient Clinic (CBOC) the Arecibo CBOC, and the Utuado, Ceiba, Comerio, and Vieques Rural Clinics. Detailed specifications and requirements are provided in the attached Performance Work Statement (PWS) Instructions to Offerors, Price/Cost Schedule, Clauses and Provisions. The contract period of performance is expected to be one (1) 12-month base period, starting on 02/01/2023, with four (4) 12-month option periods. FAR 52.212-1, Instructions to Offerors Commercial Products and Services (Addendum attached) 2. FAR 52.212-3, Offerors Representations and Certifications Commercial Products and Services (www.sam.gov registration is required to be active at time of submission of response) Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/SAM/ in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Products and Services. If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition: 1. FAR 52.212-4, Contract Terms and Conditions Commercial Products and Services (NOV 2021) (Addendum attached) 2. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (OCT 2022) Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side-by-side comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Options will be evaluated at time of award. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoters shall list exception(s) and rationale for the exception(s), if any. Questions submission shall be received not later than 10:00 AM Atlantic Standard Time (AST) on January 9, 2023 at wilfredo.perez3@va.gov. Submission of your response shall be received not later than 10:00 AM AST on January 17, 2023 at wilfredo.perez3@va.gov. The quoter shall submit a PDF version of the quote to the Government. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). Omission of any one item MAY result in a quoter package being deemed not technically acceptable. Background
The solicitation is for the provision of collection, transporting, and disposition services for Regulated Medical Waste (RMW) and pathological waste at the VA Caribbean Healthcare System (VACHS). The services will be provided at various VA facilities including the VACHS main facility, outpatient clinics, and rural clinics.

Work Details
The contractor is required to provide all labor, materials, equipment, and supervision for the collection, transportation, and disposal of RMW and pathological waste. This includes picking up RMW containers, disposing of the waste in accordance with federal and state regulations, providing clean and empty containers at the pickup locations, removing debris resulting from collection services, packing RMW according to regulations, labeling and marking packages properly, transporting RMW containers using approved methods,
incinerating pathological waste in an approved incinerator, providing monthly reports on tonnage collected and disposed of RMW for each facility.

Period of Performance
The period of performance for this service contract is from February 1, 2023, to January 31, 2024, with four 12-month option periods.

The contract will be performed at various locations including the VACHS main facility in San Juan, Puerto Rico; outpatient clinics in Ponce, Mayaguez, Guayama, Arecibo; and rural clinics in Utuado, Ceiba, Comerio, and Vieques.
