The purpose of this policy implementation comparative case study is to identify effective strategies and potential challenges associated with the implementation of state telehealth policies to prevent, treat, and/or manage cardiovascular diseases, particularly among those disproportionately affected by cardiovascular diseases. The study will inform CDC's goals to advance health equity especially among those who have experienced socioeconomic disadvantages, historical injustice, and other systemic inequalities that are often associated with social categories of race/ethnicity, gender, social position, sexual orientation, and disability. The results of the project will yield practice-based evidence in a series of audience-specific documents to inform program improvement and decision-making and enhance reach among public health practitioners.
