Background

(4) Date of invoice.
(5) Invoice number.
(6) Amount of invoice and cumulative amount invoiced to-date.
(7) Contract Line Item Number (CLIN).
(8) Description, quantity, unit of measure, unit price, and extended price of supplies/services delivered.
(9) Prompt payment discount terms, if offered.
