Document Destruction Service for US Naval Hospital, Guam & Apra Harbor Branch Clinic

Background
The U.S. Naval Hospital Guam (USNH Guam) and the Apra Harbor Branch Clinic are seeking a contractor to provide Document Destruction Services. This initiative is part of the U.S. Navy Medicine Readiness and Training Command Guam's mission to manage and properly dispose of all documentation containing patient identifiable information (PII) and patient health information (PHI) in compliance with HIPAA regulations.

The goal of this contract is to ensure secure destruction of confidential documents while adhering to necessary certifications.

Work Details
The contractor shall provide all resources including personnel, equipment, facilities, containers, and transportation for the secure destruction/shredding of confidential documents. Key tasks include:
- Providing lockable security containers (72 total) at no cost to USNMRTC Guam.
- Collecting 11 containers weekly, 72 containers on the second week of each month, and 72 containers on the fourth week of each month.
- Issuing a signed certificate of destruction upon completion, detailing the date, material destroyed, method of destruction, and signatures from designated individuals.
- Ensuring all operations staff are licensed, bonded, HIPAA certified, and NAID approved.
- Complying with record disposition schedules established by USNMRTC Guam.

Period of Performance
The contract will be performed over multiple periods: Base Year from September 1, 2025 to August 31, 2026; Option Periods extending through August 31, 2030.

U.S. Naval Hospital Guam and Apra Harbor Branch Clinic located at Naval Base Guam.
