Background
The Department of Veterans Affairs National Acquisition Center issued solicitation SPE2D1-15-R-0004 for DIN-PACS IV, a Commercial Items contract for the acquisition and implementation of a digital imaging network picture archiving and communication system.
Amendment 0009 updates solicitation language and references all required attachments and clauses.

Work Details
The contract is governed by Section 3 – Statement of Work and Functional Requirements V5 and includes:
Attachment A – Vendor Price Book V5 (Excel) where vendors must complete all fill-ins and tables 1-3 on the Discounting Info tab and provide sample invoices/quotes.
Attachment C – Modification Price Book (Excel) for future modifications, where vendors must respond to each outlined paragraph of the SOW and Functional Requirements spreadsheet.
All pricing and functional responses must be submitted in the prescribed formats.

Department of Veterans Affairs medical facilities nationwide

Bidder Requirements
1) Signed, completed Solicitation SPE2D1-15-R-0004 including all clause fill-ins and attachments.
2) Completed Vendor Price Book V5 (Attachment A) with all tabs and discount info filled.
3) Current Commercial Catalog.
4) Current Federal Supply Schedule.
5) Other than certified cost or pricing data supporting offered item prices and sample invoices/quotes.
6) Responses to each paragraph of the SOW and Functional Requirements in the Section 3 spreadsheet.
7) Small Business Subcontracting Plan if total sales exceed threshold and the offeror is a large business.
8) All exceptions to the solicitation listed separately on page 4 of the caution notice.
9) A written Contractor Code of Business Ethics.
