This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Paper copies of this solicitation will not be available.

The USPFO for Puerto Rico, located at Fort Buchanan, Puerto Rico, intends to issue a Request for Quote (RFQ) to award a single firm fixed-price contract for Dentistry Services for a base period of ten months and four option years. The evaluation factor will be de lowest price technically acceptable. The contractor shall provide all personnel, equipment, facility, tools, materials, supervision, and quality control necessary to perform Dentistry Services as defined in the PWS. The contractor shall provide services between normal operation hours from Monday through Sunday. Performance shall be at Mayag ez, Puerto Rico.

The North American Industry Classification System (NAICS) code for this work is 621210. This is a total small business set aside. The small business size standard is $7 million average annual revenue for the previous three years.

The solicitation closing date is scheduled for 23 September 2016 at 4:00 P.M. Actual dates and times will be identified in the solicitation. The Government intends to award without discussions.
