This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation 12466019Q0007 is being issued as a Request for Quotation (RFQ), to provide Log Deck Debris Removal Timber Compartment 10, for the Savannah River Site, Aiken, South Carolina. The North American Industry Classification System (NAICS) code is 562119 and the size standard is $38.5. This is a 100% Small Business Set-Aside. Pricing and technical information will be submitted to: Reid Carroll, USDA Forest Service, and NO FAXED QUOTES WILL BE ACCEPTED. E-mail quotes to recarroll@fs.fed.us. Service Wage Rates are applicable. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAM (www.sam.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically, submit only paragraph (b) from the provisions and a DUNS number. Pricing: The contractor will bid to remove debris from all compartments as a flat rate quote and not priced per hour. see amended (sow).
