This is a Special Notice for commercial items prepared in accordance with FAR Subpart 5.2, Synopsis of Proposed Contract Actions. This announcement constitutes the only Notice of Intent (NOI) to Sole Source (SS) and synopsis of proposed action. The Department of Veterans Affairs Michael E. DeBakey VA Medical Center (MEDVAMC) Network Contracting Office (NCO) 16 intends to issue and award a Sole Source single award Firm-Fixed Price type contract IAW FAR Overhaul Part 12 with a performance period of one (1) year to Terarecon Inc. located at 4309 Emperor Blvd Ste 310, Durham, NC 27703-8069 for support of government owned Terarecon PACS system at the MEDVAMC, located at 2002 Holcombe Boulevard, Houston, TX 77030. It has been determined that through initial Market Research, only 1 (one) vendor exists that provides an on-premises neuroimaging solution fully compliant with VA cybersecurity and IT policies. Competing platforms primarily operate as cloud-hosted services, which are not authorized within the VA environment due to strict data security requirements. The contract will be a Firm Fixed Price single award. The statutory authority for this non-competitive acquisition as implemented by FAR 6.302-1. Only one responsible source and no other supplies or services will satisfy agency requirements . Background
The Department of Veterans Affairs Michael E. DeBakey VA Medical Center (MEDVAMC) Network Contracting Office (NCO) 16 intends to issue and award a Sole Source single award Firm-Fixed Price type contract for the support of the government-owned Terarecon PACS system.
The contract is necessary to ensure continuous reliability of the Terarecon PACS software, which is essential for evaluating and managing conditions such as dementia, multiple sclerosis, traumatic brain injury, epilepsy, stroke, and neuro-oncology. Only one vendor, Terarecon Inc., has been identified as capable of providing an on-premises neuroimaging solution that complies with VA cybersecurity and IT policies.
Work Details
The contractor shall provide standard service and support for the Terarecon PACS system, including:
- Software licensing and support for on-premises implementation of TR AI Framework (Eureka Clinical AI), Cercare MR Neuro Advanced package FDA cleared for 1,000 runs, Combinostics cNeuro cMRI algorithm, TR AI Platform premier success per AI Algorithm, TR Connect per Interface, and TR AI framework implementation package.
- 24/7 telephone and remote support.
- On-site emergency service.
- Ongoing software updates.
- Response within 2 hours by OEM trained personnel for phone service issues; if unresolved, an on-site visit within 1 business day is required.
- Documentation of services rendered through Vendor Engineering Service Reports (ESRs).
- Compliance with all relevant guidelines from the Department of Veterans Affairs Handbook 6500.

Bidder Requirements
Bidders must demonstrate established business operations with full-time staff including a 'fully qualified' Field Service Engineer (FSE) who has completed formal training on the equipment system. Contractors must provide documentation of their personnel's qualifications including training certificates. If personnel are to work on-site at MEDVAMC, they must provide Tuberculosis vaccination documentation, flu vaccination documentation, and HIPAA compliance training certificates.
