New Contract - Polypharmacy (VA-26-00018584)

Background
The Department of Veterans Affairs (VA) aims to acquire a comprehensive software solution to address and reduce inappropriate polypharmacy. This initiative is critical due to the risks associated with the concurrent use of multiple medications, particularly for Veterans with complex medication management needs.

The project aligns with VA's commitment to enhance patient safety and reduce potentially inappropriate medications across all care settings. The solution will be implemented in the VA Enterprise Cloud (VAEC), following the Cloud First Policy established in 2018.

Work Details
The contractor will provide a software solution that includes:
1. Identification of cumulative multi-drug interactions;
2. Optimization of medication regimens;
3. Enhancement of clinical decision-making through evidence-based tools;
4. Compliance with various federal information security standards and regulations;
5. Regular updates and maintenance of the software as per the Performance Work Statement (PWS);
6. Deliverables include project management plans, monthly progress reports, training materials, and outcomes reporting.

Place of Performance
The work will be performed at locations designated by the Department of Veterans Affairs, primarily utilizing cloud services hosted in the VA Enterprise Cloud.
