Amend 005 is the fourth amendment that requires acknowledgement.
The purpose of Amendment 0005 is as follows:
1. Provide the updated solicitation document 140D0423R0008 Amend 005 to update that PPQs may be sent to the CO/CS team by email or may be provided by the Contractor with their proposal submission. Additionally, to include information regarding utilizing past performance from subcontractors, parent, sister, predecessor or other affiliate companies within Addendums to FAR Clause 52.212-1 and FAR Clause 52.212-2.

Background
This solicitation is being issued by the Department of Interior (DOI)/Interior Business Center (IBC), Acquisition Services Directorate (AQD). AQD is a Federal Franchise Fund Contracting Office that operates under the authority of the Government Management Reform Act, as amended. AQD provides contracting support to all Federal Agencies and the Department of Defense. AQD Procurement will handle the solicitation, award, contract administration, and invoice payments for this requirement. AQD is issuing this Request for Proposal (RFP) on the behalf of Health and Human Services (HHS), Indian Health Services (IHS).

Work Details
The Performance Work Statement (PWS) provided at Attachment 1 sets forth the description of services to be performed under the resulting contract. The Contractor shall be responsible for managing personnel and is responsible for providing personnel with the necessary level of expertise to support the services identified in the PWS. The Contractor shall be responsible for managing and overseeing the activities of all Contractor personnel, as well as subcontractor efforts used in performance of this effort.

Period of Performance
The projected period of performance will be February 1, 2023 – January 31, 2028, which includes a one-year base period and four one-year option periods.

Place of Performance
All work under this contract will be accomplished remotely or at the Contractor’s facility.
