**Amendment 2 - The purpose of this amendment is to provide the site visit sign-in sheet for the site visit held on 14 November. Please see B05 A00002.

**Amendment 1 - The purpose of this amendment is to notify contractors that the site visit location has been changed. We will be meeting at Crownpoint Healthcare Facility at Junction Rd 371 Rt 9, Crownpoint, NM 87313. Please meet in the conference room.

This is a Notice for Solicitation Number 75H70124R00004 for the project entitled Crownpoint Staff Quarters Replacement located in Crownpoint, NM . These construction services will be in support of the Indian Health Service (IHS) prepared in accordance with FAR Part 36.

A full scope of work, plans and specifications and Construction Wage Determination will be included in the solicitation documents.

This project will be advertised as full and open competition under North American Industry Classification System (NAICS) Code 236116 New Multifamily Housing Construction (except For-Sale Builders). The associated size standard for this procurement is $45 million.

SCOPE OF WORK: The purpose of this is to replace existing Longmark homes that are sub-standard and construct 33 new units to replace the existing Longmark homes to include demolition/abatement of twenty five (25) units and construction of three (3) Single Family homes (1 and 2 bedroom, 1 HC), six (6) Quadplex multi-family homes (1 and 2 bedroom), and three (3) Duplex multi-family homes (2 bedroom). Additional new staff quarters are needed for the non-local positions to meet the housing shortage that has impaired recruitment for "non-local" employees at the Crownpoint Healthcare Facility (CHF) in Crownpoint, New Mexico.

LOCATION OF PROJECT: The project is to be performed at the following site; Crownpoint, NM.

TERO: TERO fees do not apply.

PERIOD OF PERFORMANCE: The contract Period of Performance is 730 calendar days from date of notice of proceed.

CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be more than $10,000,000, in accordance with FAR 36.204.

CONTRACT TYPE: The Indian Health Service intends to award a Firm Fixed Price (FFP) contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 36 procedures utilizing Lowest Price Technically Acceptable evaluation criteria.
