Description

Veterans' Health Administration, Network Contracting Office 5, is issuing this request for information/sources sought notice in order to identify SDVOSBs that have the resources and capabilities to provide cranial prosthesis and cranial prosthesis services to the Washington DC VA Medical Center servicing veteran beneficiaries in Washington, D.C. and the Washington D.C. metropolitan area.

Background
The Veterans’ Health Administration, Network Contracting Office 5, is seeking Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) to provide cranial prosthesis and related services to the Washington DC VA Medical Center, serving veteran beneficiaries in Washington, D.C. and the surrounding metropolitan area.

Work Details
The contractor will provide cranial prosthesis services including initial consultation, order form completion specifying prosthesis details such as hair type (human or synthetic), color, length, curl pattern, density, cap construction, and lace material.

Services include measurements, fabrications, installation/fitting, repair services, resizing, and exchanges/returns within specified conditions. Delivery of items must occur within 30 calendar days after acceptance of a purchase/delivery order. Contractors must obtain a signature from the Veteran at delivery and maintain records without processing charges until delivery is confirmed.

The contractor warrants against defects for 90 days from acceptance and agrees to correct defects without additional cost. Repairs post-warranty are subject to VA approval with itemized fees. Contractors must notify the VA of recalls or required adjustments within specified timeframes and provide semi-annual service inspections for each Veteran served.
