Description

Contractor shall provide non-personal Companion Care Services to eligible beneficiaries authorized by the Southern Arizona VA Health Care System (SAVAHCS). Contractor shall provide all necessary personnel, equipment, transportation, and supplies associated with the full range of Companion Care Services within the terms, conditions, and provisions of this contract. The VA beneficiaries to be cared for under this contract, as referred by the SAVAHCS, shall receive the Companion Care Services after post procedure. Prior to the beneficiary release from the VA hospital, SAVAHCS will screen, verify, and refer the VA beneficiary to the contractor for companion care services. The VA providers will verify that the beneficiary no longer requires medical supervision but requires caregiver observation.

Background
The Southern Arizona VA Health Care System (SAVAHCS) is seeking a contractor to provide non-personal Companion Care Services to eligible beneficiaries. The goal of this contract is to ensure that veterans receive necessary support after medical procedures, facilitating their transition from hospital care to home care. The contractor will be responsible for providing all personnel, equipment, transportation, and supplies needed for these services.

Work Details
The contractor shall provide experienced caregivers for non-medical, private duty home care assistance and transportation from medical facilities post-procedure. Services include:
- Transportation to and from medical facilities
- Personal hygiene assistance
- Bathing if needed
- Medication reminders (without dispensing medications)
- Personal care
- Ambulation assistance
- Meal preparation
- Overnight stays as approved by SAVAHCS Social Work Chief or designee.

The contractor must ensure that all staff meet required qualifications and maintain beneficiary safety during service delivery. Performance standards include providing 24-hour advance notice for service requests with a fulfillment standard of 98%, ensuring timely arrival within a 15-minute window with a standard of 95%, and maintaining 100% compliance in beneficiary safety and staff qualifications.

Period of Performance
The contract will have a base year with four (4) option years, totaling five (5) years.

Place of Performance
Services will be performed at the Southern Arizona VA Healthcare System located at 3601 S. 6th Ave, Tucson, AZ, and at the residences of beneficiaries in Cochise, Graham, Santa Cruz, Pinal, Pima, and Yuma Counties.
