Background
The Department of Veterans Affairs, VA Northern California HealthCare System, is seeking Community Nursing Home (CNH) services for eligible Veterans within the Reno, NV catchment area as part of the Veterans Health Administration continuum of care.

Work Details
The contractor shall provide nursing home care services to meet the total medical, nursing, and psychosocial needs of VA beneficiaries. This includes ensuring CMS certification and state nursing home licensing.

Services will include room, meals, nursing care, provider visits at a rate of one visit per month, laboratory and x-ray services as needed, and other supplies commensurate with VA-authorized levels of care. The CNH must have bed capacity to accept VA referrals within 24 hours and cooperate with VA staff in referral processes. Quality oversight visits will be conducted by authorized VA representatives to ensure continuity of care.
