Cibecue Health Center – SSER Phase 2: Site Assessment   2

Background
The Indian Health Service, Phoenix Area, Whiteriver Service Unit (IHS/PHX/WRSU) is planning to construct a 55,864 gross square feet Healthcare Facility with 165 parking spaces at the Cibecue Health Center in Cibecue, Arizona. This facility aims to serve Native Americans and Alaska Natives on the White Mountain Fort Apache Indian Reservation.

The new clinic will provide various healthcare services including family practice, dental, eye care, mental health, laboratory, pharmacy, general radiology, and physical therapy. A Program Justification Document has been developed to replace the existing health center with this new facility.

Work Details
The contractor is required to prepare a comprehensive Site Assessment that includes the following components:
1. Environmental Assessment compliant with federal regulations and policies.
2. Floodplain Assessment analyzing potential floodplain conditions affecting construction and operation.
3. Archeological/Cultural Resources Survey to locate historical sites and assess impacts.
4. Geotechnical Survey for soil analysis to determine construction feasibility.
5. Legal Survey including boundary survey with CAD drawings and legal descriptions.
6. Biological Survey assessing impacts on endangered species and habitats.
7. Will Serve Letters from utility companies confirming service capacity for the new facility.
8. Traffic Impact Analysis evaluating traffic patterns related to the new clinic's operation.
9. Dry Barrel Fire Hydrant Test and Inspection to ensure adequate fire safety measures are in place.

Period of Performance
The Site Assessment shall be completed within 210 calendar days of the Notice to Proceed.
