Amendment 1 is issued to:

1) Add RFI questions and responses as of March 11, 2025, and

2) Make the following changes to the Request for SF330s (Qualifications):

a. Revise paragraph E.1. to extend recent projects to within seven (7) years,

b. Correct paragraph E.2. to add Licensed to the Structural Engineer requirement and Qualified Elevator Inspector (QEI) certification to the Elevator/Transport Specialist requirement,

c. Revise paragraph E.4. to allow for submission of previously completed Past Performance Questionnaires,

d. Extend the submission due date to March 27, 2025 at paragraph H.2., and

e. Revise paragraph H.3.d. to exclude SF 330 Part II from the page limitation and allow a smaller font for illustrations, tables, and graphics.

Background
The Department of Veterans Affairs (VA) Office of Construction and Facilities Management (CFM) Central Region is issuing this public announcement for Request for Qualifications (SF330s) (Reference 36C10F25R0014). This request is specifically for Architect/Engineer (A/E) services to conduct a Facility Condition Assessment (FCA) for Veterans Integrated Services Network 2 (VISN 2).

The purpose of an FCA is to visually inspect the technical and physical conditions of VA Medical Center (VAMC) facilities and provide a report detailing identified corrective actions. FCAs are conducted for all VAMC locations and are updated every three years.

Work Details
The contractor shall perform an independent facility condition assessment that includes tours, interviews, inspections, and research at various VA Medical Centers. The assessment will cover approximately 436 buildings across VISN 2, which encompasses about 13,347,645 gross square feet.

The specific sites include Albany, Bath, Batavia, Bronx, Brooklyn, Buffalo, Canandaigua, Castle Point, East Orange, Lyons, Montrose, New York City, Northport, St. Albans, and Syracuse. The contractor will evaluate deficiencies using a grading system where deficiencies rated 'D' or 'F' will be assigned a Risk Level based on the FCA Risk Level Matrix. The contractor will also coordinate with facility personnel to prepare for site visits and ensure access to necessary documents and areas. After conducting site visits and gathering data, the contractor will update the FCA database with pending changes and create reports summarizing findings for each facility.

Period of Performance
All work for this requirement is to be completed within 360 calendar days after the Notice to Proceed (NTP).

Place of Performance
The assessment will be performed at various VA Medical Centers located in New York and New Jersey.
