This is a COMBINED SOLICITATION/SYNOPSIS for commercial items/service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award Multi-vendor, multi-award IDIQ (indefinite-delivery, indefinite quantity type contracts) utilizing procedures under FAR Part 12. The proposed contract action is to provide non-personal professional PHYSICIAN SERVICES FOR THE BEMIDJI AREA FEDERAL FACILITIES/ LOCATED IN NORTHERN MINNESOTA in accordance with the PERFORMANCE WORK STATEMENT (PWS)

THIS SOLICITATION IS RESTRICTED TO NATIVE AMERICAN FIRMS ONLY IAW -- 25 USC. "THE BUY INDIAN ACT".

This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The Request for Proposal (RFP) number RFP- 75H70524R00001 is set-aside for Native American firms IAW 25 U.S.C. 47(a). This solicitation documents and incorporates provisions and clauses in effect through FAC 2023-05 effective 22 September 2023. It is the contractor's responsibility to be familiar with the applicable clauses and provisions.

The North American Industry Classification System (NAICS) Code for this acquisition is 561320 (PSC) is Q509- Medical INTERNAL MEDICINE.

Period of Performance Estimated base period + four (4) 1 year options.

The Government intends to award a mutiple Indefinite Deliver - Indefinite Quantity (IDIQ) type contracts resulting from this Solicitation to the responsive and responsible offerors whose conforming response is determined to provide the best value to the Government, price and other factors considered.

Background
This is a COMBINED SOLICITATION/SYNOPSIS for commercial items/service prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. The proposed contract action is to provide non-personal professional PHYSICIAN SERVICES FOR THE BEMIDJI AREA FEDERAL FACILITIES located in NORTHERN MINNESOTA in accordance with the PERFORMANCE WORK STATEMENT (PWS). The solicitation is restricted to Native American firms only IAW -- 25 USC, 'THE BUY INDIAN ACT'.

Work Details
The Contractor shall provide personnel to perform non-personal professional physician services for the Emergency Room (ER) and/or the primary care clinic (family practice, pediatrics, internal medicine clinic), or other primary care locations as identified within the Indian Health Services Bemidji Area. The physician shall be required to perform emergency trauma work and/or primary care work. The contract employees performing this service are hereafter referred to as Health Care Providers (HCP).
