Naval Medical Center Portsmouth HT9406 has a requirement for Breast implants and expanders.The vendor shall supply breast implant products and related services to NMRTC Portsmouth.The vendor will provide technical or clinical support as reasonably required to facilitate safe and effective use of the products within NMRTC Portsmouth. 1. RFI TITLE: Breast Implants and Expanders Blanket Purchase Agreement for Navy Medicine Readiness and Training Command (NMRTC) Portsmouth

2. LOCATION: NMRTC Portsmouth, Virginia

3. BACKGROUND: This is for a new BPA for the Plastic Surgery Department at NMRTC Portsmouth. The reagents will be delivered on an as-needed basis to NMRTC Portsmouth.

4. DESCRIPTION: NAICS 339113 Surgical Appliance and Supplies Manufacturing, PSC 6515 Medical And Surgical Instruments, Equipment, And Supplies. See DRAFT Statement of Work (SOW).

RFI SUBMISSION INSTRUCTIONS:

Responses to this RFI shall adhere to the following formatting and outline instructions:

• Organization
• Responder's technical and administrative points of contact (names, addresses, phone numbers, and email addresses)

3. Capabilities statement SHALL address the following:

• Contractor's clear understanding and ability to perform the scope of this requirement as detailed in the DRAFT SOW.
• Contractor's experience performing the same or similar work as detailed in the DRAFT SOW.
• Existing contract vehicles (e.g., GSA schedule) under which the responder currently offers these services, if any.
• Whether the responder provides the required services to the general public under terms and conditions similar to those offered to the Federal Government.
