The Mann-Grandstaff VA Medical Center (MGVAMC) requires the following services to be provided in a private hospital, office or clinic environment to Veterans, primarily residing in Bonner County, Idaho: Clinical services are provided to remote areas through a telehealth clinic or other arrangement. If any other services are provided in this venue (external to a VA clinic or facility), they must be associated, attached to, and coordinated by a health care delivery site located in a clinic or facility.

Proposal due date has been extended until 05/17/2021, 5:00 PM PST. Please see attached amendment 0001.

Prospective offeror's questions have been addressed and proposal due date has been extended until 05/20/2021, 5:00 PM PST. Please see attached amendment 0002.

Block 14 of amendment 0002 stated an incorrect proposal due date of 30 May 2021, 5:00 PM PST. Amendment 0003 corrects the proposal due date to 20 May 2021, 5:00 PM PST. Please see attached amendment 0003.

Proposal due date has been extended until 05/31/2021, 5:00 PM PST. Please see attached amendment 0004.

Additional questions have been addressed, Schedule of Services has been updated to reflect current POP and updated PMPM quantities, and the proposal due date has been extended until 06/10/2021, 5:00 PM PST. Please see attached amendment 0005.
