Background
The Indian Health Service (IHS) is issuing this Request for Quotations (RFQ) for the Bill Parkin (MAK) project, which involves the installation of water and sewer systems on Trust land within Clallam County, specifically for a private home owned by a member of the Makah Tribe.

The goal of this contract is to provide essential sanitation facilities that will be operated by the homeowner upon completion.

Work Details
The contractor is required to perform the following tasks:
- Mobilization and demobilization;
- Installation of solid sewer pipe;
- Installation of an underground cleanout;
- Installation of a 1,000-gallon concrete septic tank;
- Installation of a 1,000-gallon individual lift station with pump and controls;
- Installation of an effluent transport line;
- Installation of a sand mound drainfield;
- Installation of a water service line;
- Installation of a water meter assembly with pressure reducing valve.

All work must comply with applicable codes, references, and standards, including Washington Administrative Code (WAC Chapter 246-272A).

Period of Performance
The contract performance period is set for 60 calendar days from the issuance of the Notice to Proceed.

Place of Performance
91 West Place Road, Neah Bay, WA 98357-9999, located within the Makah Indian Reservation.

Bidder Requirements
Bidders must furnish performance and payment bonds within 10 calendar days after award. Contractors must comply with all insurance requirements, including workers' compensation, general liability, and automobile liability insurance. Additionally, bidders are required to have appropriate contractor licensing.
