Award Notice - Q526 (VA)

The contractor shall provide all resources necessary to accomplish the deliverables described in this Statement of Work (SOW), except as may otherwise be specified. A contracted CRC home is needed and must be able to handle up to 12 seriously mentally ill (SMI) Veterans, contractor will need to meet all VA, The Joint Commission, and Commission on Accreditation of Rehabilitation Facilities (CARF) Accreditation standards. The contractor will be a 24-hour provider for SMI patients; providing personal care, a secure and safe living housing scenario, providing for needed nutritional needs, administering required/prescribed medications, transporting patients to and from scheduled medical and mental health appointments. The CRC contractor should have a current business license, liability insurance and adequately trained staff. No other specific certifications are needed by the contractor or staff. The contractor shall be located within 10 to 15 miles of the Tuscaloosa VA Medical Center located at 3701 Loop Road, Tuscaloosa AL 35404.
