AMENDMENT THREE. Firm Fixed-Price, Non-Personal Commercial Services, Indefinite-Delivery/Indefinite Quantity Contract for Areawide Service, Support, and Maintenance Agreement for Various Types of Optometry Equipment for GPA IHS.

Background
The Indian Health Service (IHS) is issuing a Firm Fixed-Price, Non-Personal Commercial Services, Indefinite-Delivery/Indefinite Quantity Contract for Areawide Service, Support, and Maintenance Agreement for various types of optometry equipment.

This contract aims to support IHS health care facilities throughout the Great Plains Area in North Dakota and South Dakota, ensuring that essential medical equipment is maintained and operational.

Work Details
The contractor will provide comprehensive service and support for non-automated optical devices located at multiple IHS facilities.

Services include:
1. Cleaning, adjustment, and preventive maintenance as recommended by the original equipment manufacturer (OEM) and required by the Center for Medicare and Medicaid Services.
2. All parts, labor, tools, equipment, literature, transportation, and per diem necessary for maintenance are included in the bid price.
3. Removal of any device or accessory from healthcare facilities requires express written consent from the IHS representative.
4. Routine scheduled service will be performed Monday through Friday from 8 AM to 5 PM excluding weekends and holidays; corrective maintenance will be available 24/7.

The contractor must provide only new equipment and parts; no used or refurbished items are allowed. The contractor is also responsible for ensuring compliance with all applicable laws and regulations.

Period of Performance
The period of performance for this contract is from March 1, 2026 to February 28, 2031, including a base year plus four option years.

Place of Performance
The services will be performed at various IHS health care facilities throughout the Great Plains Area in North Dakota and South Dakota including Cheyenne River Health Center (Eagle Butte), Fort Thompson Health Center (Fort Thompson), Fort Yates Hospital (Fort Yates), Kyle Health Center (Kyle), Pine Ridge Hospital (Pine Ridge), Quentin N. Burdick Memorial Health Care Facility (Belcourt), Rosebud Health Center (Rosebud), Wanblee Health Center (Wanblee), Woodrow Wilson Keeble Memorial Health Care Center (Sisseton), and Yankton Service Unit (Wagner).
