AMENDMENT NUMER TWO - Pharmacist Services for the Woodrow Wilson Keeble Memorial Health Care Center-Sisseton Indian Health Service, Sisseton, South Dakota.

Background
The Indian Health Service (IHS) is an agency within the Department of Health & Human Services responsible for providing federal health services to American Indians and Alaska Natives.

The Woodrow Wilson Keeble Memorial Health Care Center (WWKMHCC), located on the Lake Traverse Reservation in South Dakota, aims to deliver comprehensive healthcare services to approximately 7,000 tribal members. This contract seeks to ensure uninterrupted pharmacist services that align with the facility's operational capacity and equipment, reflecting care typically provided in a rural civilian practice.

Work Details
The contractor shall provide up to two pharmacists who will perform pharmaceutical services compatible with the IHS medical facility’s operating capacity. Responsibilities include:
1. Processing prescriptions in the Electronic Health Record (EHR), verifying accuracy before dispensing, and documenting counseling and pertinent patient information.
2. Checking for completeness, appropriateness, allergies, and any drug interactions when processing prescriptions.
3. Ensuring all medications prescribed are on the WWKMHCC formulary or prescribed per local non-formulary policy.
4. Providing instructions to prescribing practitioners regarding medication substitutions and answering medical questions related to medications.
5. Maintaining accurate records of controlled medications and participating in Patient Centered Medical Home (PCMH) model activities.
6. Engaging with patients in a culturally sensitive manner and maintaining compliance with all applicable laws and regulations.

Period of Performance
The contract period is from September 1, 2025, to August 31, 2030, with a base period of six months followed by nine option periods of six months each.
