The purpose of this Amendment is to revise the Price schedule with a breakdown of Base Rate, Overtime and Holidays. To remove section 9.5 from the PWS in reference to Background Screens and Investigations Cost. Aslo to extend Quote due date to 06/23/2026 at 12:00 pm CST.

Background
The Department of Veterans Affairs, specifically the Network Contracting Office 16, is issuing this amendment for onsite phlebotomy services at the MEDVAMC in Houston, TX.

The goal of this contract is to provide necessary staffing services to support the medical facility's operations.

Work Details
The contractor will provide phlebotomy staffing services onsite at MEDVAMC.

The invoicing and payment will be based on the quantity of hours for services rendered. The price schedule has been revised to include:
- Base hourly rate for an estimated 29,642 hours
- Holiday rate at 2.0 times the base for an estimated 1,310 hours
- Overtime rate at 1.5 times the base for an estimated 2,328 hours.

This amendment also removes Section 9.5 from the Performance Work Statement regarding background screens and investigations cost.
