Background
The Department of Veterans Affairs (VA) is seeking to provide Community Based Outpatient Clinic (CBOC) services within the city limits of Kosciusko, Mississippi.
This initiative is part of the VA's mission to enhance healthcare access and quality for veterans, aligning with the Patient-Centered Medical Home (PCMH) model aimed at improving care coordination and patient satisfaction.

Work Details
The contractor shall provide comprehensive CBOC services, including but not limited to:
- Primary care services for veterans residing in Attala, Carroll, Holmes, Leflore, and Montgomery counties.
- Compliance with VHA performance and quality criteria, including access to care, quality of care, and veteran satisfaction.
- Staffing requirements include a Patient Aligned Care Team (PACT) consisting of primary care providers, nurse care managers, clinical associates, and administrative associates.
- The contractor must ensure adequate staffing ratios as per VA standards: 1200 active patients per full-time physician and 900 active patients per full-time mid-level provider.
- Implementation of a contingency plan for staffing during periods of absence or turnover.
- Coordination with the VA for continuity of care and communication regarding clinical issues.
- Adherence to all relevant VA policies regarding patient safety and quality assurance.

Period of Performance
The contract will be performed over a five-year period following the award date.

Place of Performance
Services will be conducted within the city limits of Kosciusko, Mississippi.
