Background
This is a combined synopsis/solicitation for commercial items prepared per the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations/offers are being requested. Solicitation N6809622Q2014-- is being issued as a Request for Quotation (RFQ). The items are for the US Naval Hospital Guam (hereinafter referred to as USNH Guam). The Laboratory Department at USNH Guam will inspect/accept the supplies delivered.

Accu-Chek Inform II Meter Kit + RF Equipment: The line item numbers, descriptions, product numbers/codes, quantity, delivery schedule, and unit of issue are detailed within the word document embedded on page 3 of this announcement.

The specific products to be delivered include Accu-chek Inform II Meter Kit + RF (Item#: 5060303001), Accu-chek Inform II Base Unit (Item#: 7671717190), Accu-chek Inform II Handheld Power Supply (Item#: 8692432160), and Code Key Reader NGBG (Item#: 4884671001). Quantities required are 16 each for all items. Equipment/supplies will be inspected and accepted at U.S. Naval Hospital, Guam. The Laboratory Department will perform the inspection and accept the supplies.
