This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR Part 13.5 for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quote is being requested, and a written solicitation document will not be issued. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2025-01 (eff.11/12/2024), VAAM Alert 24-10 (eff. 04/09/2024), and Acquisition Policy Flash 24-08 (eff. 05/22/2024). Contractor is required to be actively registered in the System for Award Management (SAM) and certified with the Small Business administrative (SBA) Veteran Small Business Certification (VetCert) as a Service-Disable Veteran-Owned Small Business (SDVOSB). Vendor quote may be considered non-compliant and rejected if the Contracting Officer is unable to verify registration and certification status. The Michael E. DeBakey VA Medical Center (MEDVAMC) 2002 Holcombe Blvd, Houston, Texas 77030 is seeking to purchase Pharmacy Hoods Semi-Annual Inspection. This solicitation is a SDVOSB set-aside. The government expects this announcement to result in a Firm-Fixed Price Single-Award contract for a base year plus four (4) option years not to exceed five (5) years. See attached RFQ

The purpose of this amendment 0001 is to provide all potential vendors with the amended extension of the close response date of the reference solicitation. See attached amendment 0001.

The purpose of this amendment 0002 is to provide all potential vendor with an opportunity of a technical site visit, date extension, and questions deadline extension. See attached amendment 0002.

Background
The Michael E. DeBakey VA Medical Center (MEDVAMC) is seeking to purchase Pharmacy Hoods Semi-Annual Inspection services. This solicitation is a Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside, aiming to establish a Firm-Fixed Price Single-Award contract for a base year plus four option years, not exceeding five years in total. The contract is intended to ensure compliance with safety and operational standards for pharmacy equipment.

Work Details
The contractor will perform semi-annual inspections and certifications of various pharmacy equipment including biological safety cabinets (BSC), compounding aseptic isolators (CAI), compounding aseptic containment isolators (CACI), containment segregated compounding areas (C-SCA), clean rooms, ante rooms, laminar airflow hoods (LAFH), alarms, and glove box/isolator areas. Inspections must comply with the latest United States Pharmacopeia (USP) 797 and 800 requirements.

The contractor will also be responsible for the inspection and installation of HEPA filters and pre-filters as necessary. The work includes:
1) Conducting inspections prior to certification;
2) Providing a maintenance and parts rider for normal hours, after hours, or emergency repairs;
3) Notifying the Contracting Officer's Representative (COR) of any issues that may prevent completion of required work;
4) Marking any failing units with signage indicating they are out of service;
5) Providing quarterly electronic reports on all completed work;
6) Ensuring all personnel conducting inspections are certified in relevant practices and have at least five years of experience in similar roles.
