The purpose of amendment 0003 is to update Lost Containers description included in the Pricing Spreadsheet.
"LIQ" has been replaced with "OxyTOTE"
Please see the updated Pricing Spreadsheet titled "Pricing Spreadsheet-SYR GAS_Final Updated".
Also, attached to this amendment 0003, on page 2 are the Loss Container categories each CLIN fall under.

All other terms and conditions of the solicitation remain unchanged.

The purpose of this amendment 0002 is to:
1. Extend the deadline for submissions to 2/11/2026 12pm local time.
2. Answer Questions posed by vendors this document is part of the modification document
3. Upload Final RFQ document labeled 36C24226R0023_Final
4. Upload Final Pricing Sheet labeled Pricing Spreadsheet-SYR Gas_Final

The purpose of this amendment 0001 is to extend the solicitation submission deadline. The reason for this extension is to allow vendor questions to be answered and reviewed by legal.

The VHA, VISN 2, Syracuse VA Medical Center require a contractor to provide medical gases to the VA medical center facilities. The Contractor shall be responsible for providing, filling, transporting, handling, servicing, maintaining, delivering, picking up and storing medical gases and cylinders for all facilities located within VISN 2, Syracuse VA medical center and VA Dental clinic catchment area. All equipment and materials required to perform on the contract shall be provided by the Contractor (other than government-furnished property).

Background
The Veterans Health Administration (VHA), Veterans Integrated Service Network (VISN) 2, is seeking a contractor to provide medical gases to the Syracuse VA Medical Center and associated facilities. The goal of this contract is to ensure a reliable supply of medical gases and cylinder rentals necessary for the operation of these healthcare facilities, thereby supporting their mission to provide high-quality care to veterans.

Work Details
The contractor will be responsible for providing, filling, transporting, handling, servicing, maintaining, delivering, picking up, and storing medical gases and cylinders. The specific medical gases required include:
- Medical Grade Oxygen (Aluminum Medical Grade Oxygen E Tanks)
- Medical O2 Gas Form D Tanks
- Medical Carbon Dioxide USP
- Breathing Air USP
- Nitrogen USP
- Nitrous Oxide
- Specialty Gas Cylinder Rentals.

The contract will be structured as a Fixed Price Indefinite Delivery, Indefinite Quantity (IDIQ) with five pricing periods:
1. Pricing Period 1: 02/01/2026 - 01/31/2027
2. Pricing Period 2: 02/01/2027 - 01/31/2028
3. Pricing Period 3: 02/01/2028 - 01/31/2029
4. Pricing Period 4: 02/01/2029 - 01/31/2030
5. Pricing Period 5: 02/01/2030 - 01/31/2031.

All medical gases must meet or exceed the standards set forth in the current edition of the United States Pharmacopoeia/National Formulary (USP). The quantities listed in the solicitation are estimates only and may vary based on the medical needs of each facility.

Period of Performance
The contract will commence on February 1, 2026, and will continue through January 31, 2031.

Place of Performance
The services will be performed at various locations within VISN 2, including the Syracuse VA Medical Center and associated VA Dental clinics.
