Willow Drive, Suite 300 Greenwood Village, CO, 80111 CONTRACTING OFFICER: Lindsey Zwaagstra Amendment 0002 Contractor shall provide all point of care (POC) analyzers, uploaders, reagents, standards, controls, parts, accessories, and any other item required for the proper operation of its POC analyzers and peripherals necessary for the generation of POC PT/INR (Prothrombin Time International Normalized Ratio) ASSAY Cost Per Kit (CPK) services. Services shall be required for a five-year ordering period, firm-fixed price blanket purchase agreement.

Background
The Department of Veterans Affairs (VA), through Network Contracting Office 19, is seeking a contractor to provide point of care (POC) analyzers and related services. The goal of this contract is to ensure the availability of POC PT/INR (Prothrombin Time International Normalized Ratio) assay services across multiple VA facilities, enhancing the quality of care provided to veterans. This contract will establish a firm-fixed price blanket purchase agreement for a five-year ordering period.

Work Details
The contractor shall provide all necessary equipment and supplies for POC PT/INR testing, including:

- Analyzers:
- Rocky Mountain Regional VA Medical Center: 11 analyzers
- Oklahoma City VA Health Care System: 15 analyzers
- Salt Lake City VA Health Care System: 17 analyzers

- Uploaders:
- Rocky Mountain Regional VA Medical Center: 11 uploaders
- Oklahoma City VA Health Care System: 15 uploaders
- Salt Lake City VA Health Care System: 9 uploaders

- Additional equipment includes docking stations (21), simulators (2), and cleaning kits (3). The Cost Per Kit (CPK) pricing must cover all costs associated with equipment use, reagents, maintenance, and training for personnel. The equipment must be new and state-of-the-art, with no remanufactured or discontinued models accepted. All necessary upgrades during the contract term must be provided at no additional cost.

Period of Performance
The period of performance for this contract is from August 1, 2025, to July 31, 2030.

Place of Performance
Services will be performed at multiple locations including the Rocky Mountain Regional VA Medical Center in Aurora, CO; Oklahoma City VA Health Care System in Oklahoma City, OK; and Salt Lake City VA Health Care System in Salt Lake City, UT.
