RFQ Follow-on VA SAC National Dental Laboratory Products & Services   2

Background
This is a Request for Quotations (RFQ) for the VA SAC National Laboratory Products and Services Follow-On. The U.S. Department of Veterans Affairs (VA), through the Veterans Health Administration (VHA), operates the nation’s largest integrated direct health care delivery system. VHA’s health care delivery network includes 170 VA Medical Centers and 1,065 community-based outpatient clinics (CBOCs). VA is organized into 19 geographically defined Veterans Integrated Service Networks (VISNs). Although policies and guidelines are developed at VA headquarters, and applied throughout the system, management authority for basic decision making and budgetary responsibilities are delegated to the VISNs.

Work Details
The Contractor shall provide all resources necessary to accomplish the requirements described in the Statement of Work (SOW). The Contractor shall provide dental laboratory services outlined in this SOW. The dental laboratory services that may be authorized under this contract include fabrication of various dental products such as crowns, bridges, dentures, and other dental prostheses.
The contract period is one base year with four 12-month option periods.

Period of Performance
The Period of Performance is one (1) base year period with four(4) 12-month option periods.

Place of Performance
Place of performance is at the contractor’s facility.
