THE PURPOSE OF AMENDMENT 0001 SEE SF30 AMENDMENT 0001 FOR 36C24722Q0065-VISN 07: ARTIFICIAL LIMBS
Background
This solicitation is issued under procedures. The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Regional Procurement Office (RPO) Eastern Region provides prosthetic limbs and related services to veterans eligible for VA health care services. The acquisition aims to supplement VA in providing comprehensive prosthetic care and support to veterans in need. The VA intends to solicit proposals and award multiple Blanket Purchase Agreement(s) (BPAs) within the VA Medical Center’s (VAMC) Veterans Integrated Service Network (VISN) 07 jurisdiction under the authority of Title 38 U.S.C. Section 8123.

Work Details
The contractor shall fabricate all prosthetic limbs in strict conformance to the prosthetic prescription provided by the VA Amputee Clinic Team. The contractor shall not add or substitute components or alter the patient’s limb prescription without prior approval and written authorization from the COR or local area VAMC Point of Contact (POC).
The solicitation includes commercial items only, and products that are not commercial items are considered experimental and shall not be authorized under this solicitation. The contractor shall have a full-time American Board for Certification (ABC) in Orthotics, Prosthetics & Pedorthics or Board of Certification/Accreditation International (BOC) certified prosthetist(s) on staff at each proposed service location throughout the contract period.

Period of Performance
The ordering period is for five years beginning from the Date of Award through 60 months thereafter, unless terminated earlier pursuant to the terms of the agreement.

Place of Performance
The work required under this contract shall be conducted at the facility locations included in the contract. All Contractor facilities as well as other fabrication facilities such as commercial central fabrication facilities that will be utilized for this Contract shall be listed in the proposal.
