Background
The Veterans Healthcare Administration Salt Lake City Health Care System (VHASLCHCS) is seeking to procure ostomy and gastrostomy supplies through an Indefinite Delivery Indefinite Quantity (IDIQ) contract. This procurement aims to provide essential medical supplies to veterans in Utah and parts of neighboring states, ensuring they receive the necessary care and support for their ostomy needs.

The contract will facilitate the delivery of these supplies directly to veterans or through designated VA personnel, enhancing patient care and service delivery.

Work Details
The contractor will provide a range of ostomy and gastrostomy supplies as outlined in the Ostomy Supplies List, which includes both Standard and Custom items. The estimated quantity is sufficient for approximately 175 veterans, with each item priced to cover all associated costs including delivery and patient care services.

The contractor must stock Standard items and be able to supply Custom items upon request. Key requirements include:
- Delivery methods: direct mail, pickup at contractor's location, local delivery within a 30-mile radius of the VA Medical Center, or delivery to VA Enterostomal Nurses.
- All products must be new OEM medical supplies; gray market items are not acceptable.
- The contractor must maintain an established business within the Salt Lake City metropolitan area.

Period of Performance
The contract will have a Base Year plus four (4) one-year options.

Place of Performance
Supplies will be delivered primarily to veterans in Utah, with some services extending to parts of Nevada, Wyoming, Idaho, and Montana.

Bidder Requirements
Bidders must be an OEM, authorized dealer, distributor, or reseller for the proposed medical supplies. They are required to provide documentation verifying their authorization from the OEM. Additionally, if applicable, Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) must be VetCert verified at the time of quote submission and award.
