Background
This is a Request for Proposal for Prescription Eyeglasses. The acquisition will result in a single-award Indefinite Delivery Indefinite Quantity (IDIQ) Contract. The acquisition is 100% set aside for Service Disabled Veteran Owned Small Businesses (SDVOSBs). The contract is intended for use by VA Southern Nevada Health Care Systems (VASNHCS) and VA Palo Alto Health Care Systems (VAPAHCS) located within VISN 21. The VA Health Care Systems’ breakdown is included in paragraph 2.4.
VASNHCS and VAPAHCS furnish an estimated 33,000 pairs of eyeglasses annually to eligible veterans.

Work Details
The contractor shall provide completely assembled eyeglass frames placed in standard alignment and include mounting of lenses. All items provided by the contractor shall include a minimum of one-year warranty. The contractor shall deliver all items no later than SEVEN (7) BUSINESS DAYS after receipt of both the prescription consult and purchase order issued by Government authorized personnel against the contract. The contractor shall monitor their stock and propose a substitute before that style of frame becomes unavailable.

Period of Performance
The contract period is from 08/01/2022 through 07/31/2027.

Place of Performance
The geographic locations for performance are VA Southern Nevada Healthcare System, North Las Vegas, NV, and VA Palo Alto Healthcare System, Palo Alto, CA.
