Background
The Department of Veterans Affairs is seeking to provide Peritoneal Dialysis Kits tailored to the specific needs of patients. The goal of this contract is to ensure that patients receive the necessary supplies for at-home dialysis on a monthly basis, enhancing their quality of care and supporting their health management from home.

Work Details
The contractor shall deliver Home PD Bundles/Kits and Equipment/Cycler rentals as specified below:

- **Home PD Bundle/Kit**
- Estimated Quantity: 296 EA per year
- Manufacturer Part Number (MPN): KMSA4900

- **Equipment/Cycler only rental**
- Estimated Quantity: 25 EA per year
- Manufacturer Part Number (MPN): KMSE1594

- **Equipment/Cycler only rental for inpatient use at Hines VAMC**
- Estimated Quantity: 12 EA per year
- Manufacturer Part Number (MPN): KMSE1594

- **Shipping Charges for rental**
- Estimated Quantity: 1 EA per year

This contract will have multiple option periods extending through five years, with a ceiling amount of $7,200,000.00 for the entire contract duration.

Period of Performance
The contract will be effective from January 1, 2025, through December 31, 2029.

Place of Performance
Deliveries will be made to the patients' places of residence as well as Hines VAMC.
