Custom Surgical Packs - AMENDMENT TO CANCEL   3

Background
The Department of Veterans Affairs (VA) is seeking to procure custom surgical packs for the Salisbury VA Medical Center and its associated clinics. The goal of this contract is to provide a Custom Sterile Procedure Pack program that meets the surgical needs of the VA Health Care System. These custom packs are essential for various surgical procedures and must be delivered on an as-needed basis. The current procurement method has surpassed the micro-purchase threshold, necessitating a more formal acquisition process.

Work Details
The contractor will supply custom surgical procedure packs that include assembly and sterilization services. The types of surgical packs required encompass a range of procedures including Cataract surgery, Knee surgery, Hip surgery, Arthroscopy, Chest procedures, Ear Nose and Throat surgery, Endourology surgery, Hand surgery, Robot-assisted surgery, Podiatry surgery, Vascular surgery, Endovascular surgeries, and Minor/Clinic procedures. Each pack must include necessary supplies for these procedures and adhere to strict quality control standards set by the FDA and ISO certifications. Specific requirements include:

- Individual lot numbers for tracking
- Compliance with sterilization standards validated by AAMI
- Packaging that prevents deterioration during shipping
- Latex-free components unless specified otherwise
- Clear labeling of all components including expiration dates
- A minimum shelf life of ninety (90) calendar days upon delivery.

Additionally, the contractor must ensure timely deliveries within three business days for standard orders and twenty-four hours for emergency requests.

Period of Performance
The contract will have a base year starting from November 1, 2025, to September 30, 2026, with four optional renewal years extending through September 30, 2030.

Place of Performance
Deliveries will be made to the Salisbury VA Medical Center located in Salisbury, North Carolina.
